New Patients : Please call to be placed on a wait list for Dr. Patel or Dr. Liu. Once an appointment is booked, you will need to fill out an intake form, review our patient responsibilities agreement as well as consent to use virtual Care Tools. You can not book regular appointments until the doctor and you have agreed to a family doctor-patient relationship.
